AGREEMENT

THIS AGREEMENT, is between SUFFOLK COMMUNITY COLLEGE (COLLEGE), having its principal administrative office at 533 College Road, Selden, New York 11784-2899, a chartered Community College (pursuant to New York State Education Law) under the sponsorship of the COUNTY OF SUFFOLK (COUNTY), a municipal corporation of the State of New York, having its principal office at the County Center, Riverhead, New York 11901, and ____________________(CONSULTANT), a professional corporation organized under the laws of New York State, having its principal office at _______________________________.

The parties hereto desire to make available to the COLLEGE planning, design, and supervision services for Capital Project No. 21_____________________________________________
